Georgia Pediatric Program (GAPP)


One of our foundational programs is participating in the Georgia Pediatric Program (GAPP) specifically designed to aid parents with assistance of the basic and fundamental everyday needs. This Medicaid-funded program specializes in skilled and unskilled private In-Home Care for children under 21 years of age based on medical necessity determination. Medicaid is a medical assistance program that helps many people who can’t afford medical care pay for some or all of their medical bills. Medicaid is administered by the Georgia Department of Community Health (DCH) and pays medical bills with State and Federal tax money.

 

The great thing about this program is there is NO WAIT LIST.​

Our services are a great solution for families that want professional service for their child, who may be in need of assistance with ADL's and/or their everyday living arrangements. 

These are services offered by GAPP for children or young adults in need of in home care:

- Medication Administration

- Hygiene Care

- Bathing

- Dressing

- Take Vital Signs

- Wheelchair assistance

- Collect Treatment Plan Information

- Trach Care
   incl. changing and cleaning

- Gastrostomy Tube Care

   incl. tube Feeding, Cleaning, Changing

- Bagging via Trach or Mouth

- Respiratory Treatment

- Suctioning

- Turn or Reposition assistance

- Change Sheets and Bed Linens

- Observation

- Communicate issues to Nurse/Doctor

- Assistance with Daily Living Activities

Private In-Home Care for Children

​

This program specializes in skilled and unskilled private In-Home Care for children under 21 years of age based on medical necessity determination. This service can be paid through private insurance or self-pay.

 

This program is not a wavier service, however, it will allow your child to receive services immediately following approval. Our customized Intake and Evaluation takes up to 30 days for final approval and is designed to allow parents to participate in developing a service program specific to the needs of their child.  â€‹

Private In-Home Care Benefits

Our Professional Staff are all certified and/or licensed CNAs, LPNs or RNs. Our professional Services may included but are not limited to:

 

- Medication Administration

- Hygiene Care - Bathing, Oral Care, etc.

- Feeding - Assistance with Feedings, G-tube Feedings

- Turn or Reposition Assistance

- Transfers

- G-tube - Care and Cleaning

Nebulizer, Vest, and CPT Treatments

- Emergency Safety Protocol

- Assistance with Therapy Exercises at Home

     - ROM Exercises, Gait Trainer, Stander, etc.

Our services are a great solution for families that want a professional service for their child, who may be in need of assistance with ADL's and/or their everyday living arrangements. This program gives the caretaker assistance with the care for their child on a daily basis. From help out of bed, to grooming and dressing assistance to a nutritious breakfast, assistance can be there every step so the day gets started right. Then to a good night’s rest. Our professional staff can also assist with evening hygiene routines  and daily ROM exercises/ in and out of therapy equipment to ensure a restful night. This program can be beneficial to a healthier lifestyle for both the child and the parent.

Private In-Home Care for Adults


We offer a variety of adult in-home care for adults over 21 to receive assistance.

Adult Personal Care and Companion Services


We offer personal care services that truly help you maintain a quality of life that keeps you available for your life and community. Our services include help with:

 

  • bathing

  • toileting

  • grooming

  • shaving

  • mouth care

  • dressing

  • eating

 

Personal care assistants also help clients move about in the home or transfer between bed, chair or toilet.

 

Companion or sitter services include:

  • transportation and escort services

  • meal preparation and serving 

  • household tasks essential to cleanliness and safety


These services are available through our self-pay program.
