At Heritage Health Partners, one of our foundational programs is participation in the Georgia Pediatric Program (GAPP), a Medicaid-funded initiative designed to support families by providing in-home care for children and young adults under 21 years of age who require medical or personal assistance.
GAPP is specifically aimed at helping parents meet the basic and fundamental everyday needs of their children. The program offers both skilled and unskilled in-home care based on a medical necessity determination. Medicaid, administered by the Georgia Department of Community Health (DCH), is a medical assistance program that helps families who may struggle to pay for healthcare cover some or all medical expenses.

- Assessment & Onboarding – Our team works with your family to assess your child’s needs. Service approval typically takes up to 30 days.
- Personalized Care Plan – Once approved, your child immediately begins receiving care tailored to their needs.
